A Paradigm Shift in Brugada Syndrome Management: The BRAVE Study

What is Brugada syndrome?

Brugada syndrome (BrS) is a heart condition that changes the way the heart's electrical system controls the heartbeat. Most people feel fine most of the time, but dangerous heart rhythms can cause fainting or, in rare cases, sudden death. Because it can run in families, doctors may recommend heart tests for relatives as well.

Research shows that rare genetic variants in the SCN5A gene, which controls the heart's sodium channel, as well as common noncoding variants in the same region, are strongly linked to this condition, increasing the risk of dangerous arrhythmias. With appropriate treatment and regular monitoring, the risks linked to this condition can be significantly reduced. Dr. Koonlawee Nademanee and colleagues recently published the groundbreaking BRAVE trial, the first randomized study testing catheter ablation for Brugada syndrome patients with ICDs. With nearly a 70% reduction in ventricular arrhythmias compared to standard care, with over 80% of patients remaining VF-free after a single procedure, these findings mark a major step beyond ICD dependency, demonstrating ablation as a proactive, life-saving therapy.

This achievement brings Thailand at the forefront of global cardiac research and sets a new benchmark in managing Brugada syndrome. For patients, BRAVE transforms hope into evidence; for clinicians, it demonstrates what is possible in arrhythmia care. Under Dr. Nademanee’s leadership, The Arrhythmia Center at Bumrungrad Hospital in Bangkok, Thailand aims to be a global center of excellence in comprehensive cardiac care, ranging from arrhythmia and heart failure to cardiogenetics, structural heart disease, and preventive cardiology.

Dr. Nademanee notes that ongoing investment in advanced technology and research is key to improving patient care. His vision positions Bumrungrad as a tertiary referral hub across the Asia-Pacific region, combining top-tier clinical treatment, global collaboration, and innovation in clinical care.

She is President of the Healthy Longevity Medicine Society and Head of the Visionary Board at the Longevity Science Foundation.

Dr. Brian Kennedy is a Distinguished Professor of Biochemistry and Physiology at the Yong Loo Lin School of Medicine, National University of Singapore. He directs the Healthy Longevity Translational Research Programme and the Bia-Echo Asia Centre for Reproductive Longevity and Equality.

PhD Christopher Mason,

Dr. Christopher Mason is a Professor of Genomics, Physiology, and Biophysics at Weill Cornell Medicine in New York, where he also directs the WorldQuant Initiative for Quantitative Prediction. His work advances the understanding of genetic mechanisms underlying health and longevity.

Dr. Claudio Viecelli is a molecular and cellular biologist who received a PhD in muscle physiology from ETH Zurich. He demonstrated how ubiquitously available technologies (e.g., smartphones) can be leveraged to obtain mechano-biological descriptors of resistance exercise. He has published multiple reviews highlighting the importance of resistance exercise and its contributions to health and longevity, discussing the roles of resistance exercise descriptors and the influences of age, sex, and genetics. After his PhD, he was a postdoctoral researcher at ETH Zurich, where he developed a motorized resistance exercise machine to examine the potential for personalizing resistance exercise. In 2024, he joined Keyoniq Switzerland AG as Chief Scientific Officer.
