Introduction The prostate is a gland found only in males. Its function is to produce seminal fluid. Of chestnut shape, it wraps around the upper part of the urethra and cannot be seen or felt externally (outside the body). is a common disease and its occurrence is often related to age. Benign Prostatic Hyperplasia (BPH) is a condition in which the prostate gland is abnormally large. Wrapping around the upper part of the urethra, when enlarged, the prostate may press on the urethra and make it narrower, causing patients to experience urinary obstruction. An enlarged prostate may also cause the muscular wall of the bladder to thicken as it needs to work harder to force the urine through the narrowed urethra. What are the treatment approaches?
The treatment approaches for enlarged prostate to be considered are based on the symptoms, not on the prostate size. Even if the prostate is very large but the patient has no symptoms, it is not necessary to take medication. The Urology Center provides the following care for over 5,000 patients with enlarged prostate each year: Change water drinking habits: With mild symptoms, patients are advised to reduce the water intake. It’s also good to reduce caffeinated and alcoholic beverages.
Treatment with medications: There are currently 3 main groups of drugs:

Third is the Saw Palmetto berry extract, which helps reduce inflammation and makes urination easier. However, it may not be as effective as the other medications.

Surgical treatment:
In some cases, treatment with medications may not help improve symptoms or causes too many side effects. In other cases, there may be recurring complications in the urinary tract such as infection, high level of post-void residual, causing kidney function to worsen or blood in the urine. Photoselective Vaporization of Prostate (PVP)
Photoselective Vaporization of Prostate (PVP) uses the laser energy to quickly vaporize and precisely remove part or all of the prostate. It may be cut only partially or completely. The patient may receive local, spinal, or general anesthesia for the surgery which takes approximately 20-50 minutes.

Holmium laser enucleation of the prostate (HoLEP)
The HoLEP procedure uses pulses of laser beam to remove the tissue from the inside of the prostate blocking the urethra. An approach that can be used on prostate glands of all sizes, it is an effective treatment option offering long-term results, with reduced risk of bleeding and possibly a tissue diagnosis of cancer.

Water Vapor Therapy
In this procedure, a device is inserted into the urethra and then sterile water vapor injected into the enlarged areas of the prostate causing the blocking cells to die. The dead cells are then removed through the body's natural healing response, allowing the urethra to open and urine to flow freely. Considered convenient and easy, it is a procedure with few complications. Bumrungrad Hospital was the first in Thailand to bring in the Rezum technology to treat enlarged prostate in 2021 . The hospital’s Urology Center has been certified by Boston Scientific of the U.S. as the Center of Excellence in enlarged prostate treatment with the Water Vapor Therapy, first in Asia. It has treated 276 cases or about 30% of enlarged prostate in the country with water therapy. (July 2024 data)

Treatment with UroLift
A minimally invasive treatment for enlarged prostate, UroLift is performed under local anesthesia to widen the urethra. In the procedure, the urologist places a device through the obstructed urethra to access the enlarged prostate, and then delivers tiny implants made of medical grade stainless steel and nitinol , an alloy with superelasticity, to lift and hold prostate tissue out of the way, thus opening the urethra permanently. The procedure takes only 15-30 minutes. Bumrungrad Hospital is the first in Thailand to use Urolift in 2024 in treating enlarge prostate.
